There have been some problems submitting claims directly at the pharmacy. To get reimbursed simply fill out the appropriate form (see links below), and send it and your original prescription to the Greenshield (address provided on forms).

For information personal claims (how much will be covered), contact Greenshield’s Customer Service Centre at 1-888-711-1119, and ask for a claims simulation. The automated voice system can be difficult to navigate, so just hit “0″ for the operator.

Most claims can be processed instantly, after creating your personalized Green Shield account. Your policy number is TGA[your student number]-00, and the group policy number is 11111.  Remember to omit the first ‘zero’ in your student number. Want to get the HPV vaccine? – Why wouldn’t you? Safety first my friends.

There are currently two vaccines for HPV – they are Gardasil and Cervarix. Both of these vaccines are included in the Trent Grads benefit plan, however this does not confirm that you are necessarily eligible for coverage. To find out if you are eligible, contact Greenshield’s Customer Service Centre at 1-888-711-1119. An agent will run a claims simulation to find out the exact dollar amount the plan will cover, which in turn will give you a better idea about how much you can expect to pay out-of-pocket. Remember that HPV vaccines do have certain age eligibility criteria. For example, for Ontario residents the plan will ONLY cover plan members who are between the ages of 14 and 26.

General Information:

The 2011-2012 academic year represents the inaugural year of Trent University’s independent Graduate Student Health Benefits Plan. Coverage is provided by Greenshield (www.greenshield.ca), and the health plan is co-administered by the Canadian Federation of Students (www.cfs-fcee.ca) and the Trent University Graduate Students’ Association (GSA).

The GSA has appointed Beth Evans as the Health Benefits Coordinator to be a liaison between graduate students and these organizations. This will allow for prompt feedback on all questions regarding the plan and make the process of claiming medical expenses as fast and easy as possible.

STATEMENT OF PRIVACY PROTECTION

The Trent Graduate Students’ Association uses the student number, name, gender and date of birth of our members for the purposes of enrolling students in the Health and Dental Benefits Plans. The Trent Graduate Students’ Association forwards this identifying information on a confidential basis to Green Shield who will use it to confirm membership and process claims. In the event that the student has alternative coverage and waives coverage under the plan, the information will be used to withdraw the student from the Trent Graduate Students’ Association health benefits plan. Students are encouraged to contact the Trent Graduate Students’ Association with any questions or concerns. The health benefits brochure is designed to outline the benefits for which you are eligible and does not create or confer any contractual or other rights. All rights with respect to the benefits of an Insured will be governed solely by the Group Master Policy issued by Morneau Morneau Shepell Ltd.
